Submitting New Forms when there is a change in care

When a Veteran finally is awarded his pension and then decides he wants to move to another facility. Or move back home with in home care. (same care being provided)

Besides a new Care Expense Statement do I also needs a new 21-2680?  

Anything else? Thanks just want to make sure I don’t mess this up for our clients.

I recently had a conversation with the Philly Pension Center on this very topic because we’d been receiving rejects when submitting “facility change only” information to them.

I was told that VA is requiring, at a minimum, a new 8416 as well as the facility letter or care giver letter.  To be on the safe side, we are also going ahead and doing either the 0518 or 0516 as well.

If he’s already rated aid and attendance or housebound, a new 2680 is not required.
