TPR-VA Fiduciary Checklist for an Initial Appointment

The following list of documents and information will be required to be reviewed during the field examination that will be conducted in the near future. In order to assist with having a productive appointment and addressing all questions at the time of the field examination, please bring the items noted below with you:

  • Most recent bank statement for all of the beneficiary’s accounts to include how the bank account is titled and the most current balance.
  • Copy of Burial Trust. VA Should have NO record of any Trust
  • Copy of all trusts (Family and Living). VA Should have NO record of any Trust.
  • Two character witnesses are required to be provided for the proposed fiduciary, to include the complete address, telephone number, occupation, relationship, and length of time known for both character references.
  • If the beneficiary owns their own home, we need the approximate value, balance owed, and if there is a VA Home Loan on the residence. VA should have NO record of a home.
  • Copy of latest tax statements on the property (to verify real estate value and amount of property taxes paid). VA should have NO record of a home.
  • General idea on how much is spent on all of the beneficiary’s expenses on a month basis.
  • If the beneficiary owns a vehicle, we will need info such as the description, approximate value, balance owed, etc. VA should have NO record of any vehicles.
  • If the beneficiary drives a vehicle, please bring proof of valid license and insurance.
  • Next of kin information to include name, address, and telephone number.
  • We will need to know where the beneficiary receives medical care and how many times the beneficiary was hospitalized over the past year.
  • Verify names, addresses, and phone numbers of any dependents that do not live with the beneficiary. There shouldn’t be any dependents.
  • Verification of private life insurance.
  • Copy of most recent credit card statements for the beneficiary. Never give credit card statements to anyone.
  • A copy of court documents, if a guardianship or conservatorship has been filed by the court.
  • The proposed fiduciary should complete and fax or mail back the attached VA Form 21-0792, as soon as possible, to further expedite the field examination process.
  • Bring a list of the type and amounts of all monthly income.
  • Please be advised that the proposed fiduciary will have a credit report and criminal background check completed on him/her prior to being appointed as the fiduciary.
  • Please be advised that an accounting may be required. An accounting is the fiduciary’s written report of his/her management of a beneficiary’s income and estate. It consists of a beginning inventory, an itemization of income and expenses, and an inventory of assets remaining at the end of the accounting period
